About the Job The Case Manager is responsible for coordinating effective and efficient day to day client services ensuring delivery of flexible, tailored care which maximizes available community resources, support, and funding options. This responsibility includes adherence to Home Care legislation. The role will need to maintain strong and long-term relationships with with clients, family members, healthcare professionals and external stakeholders in order to provide safe, quality care in line with client goals and organizational philosophy. Care is underpinned by a philosophy of independence, flexibility and choice.
About You - Bilingual in Cantonese/Good English (Brisbane Southside) or Mandarin/Good English (Brisbane Northside, such as Northlake, Caboolture).
- To succeed in this role, you must be a respectful people-person with strong communication, compassion, empathy, relationship management skills.
- Applicants with the following capability would be ideal: Clinical background with an understanding about the elderly and the aged care industry.
- Two years+ experiences in Sales/ Business Development/ Case Management/ Healthcare/ Case Management.
- Working rights required for this role, AU citizen or PR would be preferred.
